WellMed’s Primary Care Team has one goal in mind: to provide the best health care to you, our patients and your families. Our coordinated team approach allows our doctors and medical staff to review and communicate your needs to specialists, nurse practitioners, physician assistants and other medical support staff. You are not just a patient of your primary care provider. You are a patient of the entire primary care team.
Primary care refers to the type of care you receive in the doctor’s office. The Primary Care Physician (PCP) is your chosen doctor.
Your relationship with your PCP is very important. The PCP is the first point of contact to help you stay healthy, diagnose and treat illnesses and generally be an advocate and guide for better health. As a leader of your health care team, the PCP will coordinate the care by providing pre-authorization and referrals when needed. The PCP will also help you with your overall health care, including preventive care, health education, disease management and medication education.
Your PCP is in charge of your overall health care. However, there may be times when you will be scheduled to see a Nurse Practitioners (NP) and Physician Assistants (PA) instead of your PCP. NPs and PAs are licensed health care professionals and are considered an extension of your PCP. NPs and PAs are available in most of our clinics and see patients regularly. In addition to communicating your health care needs with your PCP ensuring you get the best treatment, in most cases they can prescribe medication, order tests, and refer patients to specialists.
If you are diagnosed by your PCP with a chronic illness, you may have the opportunity to spend time with a Health Coach after your regularly scheduled PCP visit. A Health Coach is a registered nurse who works directly with your PCP and in collaboration with WellMed’s Disease Management program providing you with the tools and education you need to effectively live with your chronic illness.
Health Coaches provide coaching for self-management support, focused education for your specific disease needs, medication and treatment options, and identify additional resources to assist in managing your disease such as social workers, diabetic testing supplies and even diabetic shoes.

When you need more specialized attention, your PCP will coordinate and communicate your care plan with a WellMed network Specialist. The Specialist will communicate results and recommendations back to your PCP so you and your physician can determine the best course of action.
A Hospitalist is a physician who works in the hospital setting. If you are admitted to the hospital, a hospitalist will manage your care in conjunction with your PCP. Our hospitalists can access your electronic medical records at any time so you get the right tests at the right time. All care plans are communicated between the hospitalist and your PCP.
When you are admitted as a patient into a hospital, we assign a personal Case Manager to help you through every step of your health care process.
Your Case Manager will communicate with your PCP, hospitalist, hospital personnel and specialists. He or she will coordinate your medical needs during your hospital or skilled nursing facility stay, all the way through discharge. Your Case Manager will also help you with any necessary paperwork and facility transfers.
