Due to the high volume of appointment requests for the COVID19 vaccine, it is critical that appointments be kept as scheduled. However, we understand that there are situations when that is not possible, such as severe illness, hospitalization or a family emergency. If you experience such a situation, which prevents you from keeping your scheduled appointment, please complete the form below to request an alternate appointment.

Please be aware:

  • Appointments cannot be rescheduled for convenience purposes
  • Appointments cannot be rescheduled at a different location
  • Appointments cannot be rescheduled for anyone other than the originally scheduled patient
  • We will do our best to accommodate your requested reschedule date, based on availability

We cannot accommodate same-day requests (You can’t ask on the day of your appointment to be rescheduled).
