Choosing a primary care provider is one of the most personal healthcare decisions a person can make. Beyond credentials, certifications, and clinic locations, patients are often searching for someone who will truly listen, take the time to understand their concerns, and walk alongside them through every stage of life. For many patients, that search leads them to Dr. James Holmes Dinn IV.
Dr. Dinn is a board-certified internal medicine physician and the Director of Ambulatory Education. While his credentials reflect years of training and academic achievement, what most defines
Dr. Dinn’s care is something far more human: his belief that effective medicine begins with listening.
At the heart of his practice is a philosophy he shares openly with patients and trainees alike: “The person is always separate from the disease.” For Dr. Dinn, that idea is not just a guiding principle—it is the foundation of how he practices medicine every single day.
Dr. Dinn’s journey into medicine began long before medical school. Growing up, he witnessed his brother navigate life with type 1 diabetes and the challenges that often come with managing a chronic disease.
“I watched him struggle to find a physician who would listen, see the whole picture, and truly partner with him,” Dr. Dinn says. “Appointments often felt rushed or transactional, and that experience stuck with me.”
Rather than discouraging him, those experiences gave Dr. Dinn clarity and direction. He decided early on that if he pursued medicine, he would strive to be the physician his brother needed—someone who valued conversation, understanding, and collaboration as much as lab values and test results.
“I wanted to be the kind of doctor who hears the story, not just the numbers,” he explains. “Because health is so much more than what shows up on a chart.”
A South Texas native, Dr. Dinn knew early that he wanted to train and practice in the region he calls home. He completed his master’s degree at the University of North Texas Health Science Center before attending the University of the Incarnate Word School of Osteopathic Medicine.
He then stayed in the region to complete his internal medicine residency at the Texas Institute for Graduate Medical Education and Research. During residency, Dr. Dinn quickly distinguished himself not only as a skilled clinician, but also as a thoughtful leader and mentor. He served as Chief Resident and was honored as Resident of the Year—recognition that reflected both his clinical excellence and his ability to lead with empathy.
Today, Dr. Dinn serves as Director of Ambulatory Education, a role that allows him to continue caring for patients while training the next generation of physicians. Teaching, for Dr. Dinn, is deeply connected to patient care.
“When you’re responsible for teaching evidence-based medicine to residents, it keeps you sharp,” he says. “But more importantly, it ensures that both my patients and my trainees benefit from the most current and thoughtful approaches to care.”
Early in his training, Dr. Dinn considered family medicine, inspired by a mentor he deeply respected. However, as he progressed through clinical rotations, he found himself increasingly drawn to the complexity of adult medicine.
“I realized I was especially passionate about caring for adults with multiple chronic conditions,” he says. “Internal medicine allowed me to focus on complexity while still building long-term, meaningful relationships with patients.”
That mix of intellectual challenge and continuity of care continues to be one of the most rewarding aspects of his work.
Dr. Dinn describes his approach to patient care as individualized, holistic, and deeply personal.
“My job is to treat the person first and then the diagnosis,” he explains. “The disease may guide our medical decisions, but it should never define the person sitting in front of me.”
Rather than moving quickly through symptoms, Dr. Dinn takes time to understand each patient’s life beyond the exam room—their routines, stressors, support systems, and long-term goals. He then builds treatment plans that combine preventive care, lifestyle medicine, obesity medicine, diabetes management, and general internal medicine in a way that feels realistic and sustainable.
When asked what matters most in building trust with patients, Dr. Dinn’s answer is simple.
“Trust starts with listening,” he says.
Many patients come into appointments expecting to be rushed or spoken to rather than spoken with. Dr. Dinn intentionally works against that dynamic by practicing active listening and what is known as closed-loop communication.
“I summarize what I’ve heard and check that I understood correctly,” he explains. “That moment helps patients feel truly heard and involved in their care.”
Dr. Dinn strongly believes that patients should be active partners in their healthcare decisions. He explains options clearly, outlines risks and benefits in plain language, and makes evidence-based recommendations—then invites patients to decide what aligns best with their values and goals.
“The plan should feel like a partnership, not a prescription,” he says.
As an academic internal medicine generalist, Dr. Dinn treats a wide range of adult health concerns and enjoys serving as a true ‘one-stop shop’ for his patients whenever possible.
His clinical interests include chronic disease management, diabetes care, preventive and lifestyle-focused medicine, obesity medicine, and large joint and bursal injections—procedures he feels are becoming less common in primary care. As an osteopathic physician, he also provides osteopathic manipulative treatment (OMT), offering patients an additional, hands-on approach to care.
Dr. Dinn is board-certified in internal medicine and is actively working toward additional board certifications in lifestyle, obesity, and pain medicine. He also has a strong interest in diabetology and clinical psychiatry, dedicating specific clinic time to patients with these complex needs.
Patients often appreciate the thorough and team-based approach used in Dr. Dinn’s clinic. New patients are typically first seen by an internal medicine resident who performs a detailed history and physical exam.
That information is then reviewed with Dr. Dinn, who independently examines the chart, reviews the plan, and meets with the patient to ensure care is current, safe, and tailored to their individual needs. This model allows patients to benefit from multiple physicians’ perspectives without losing continuity of care.
Several factors drew Dr. Dinn to WellMed, but time was one of the most important.
“Seeing fewer patients per day allows me to spend more time with each person,” he explains. “That leads to better conversations and better care.”
He also values the strong, team-based support available in the clinic, including nurses, medical assistants, social workers, and community-based programs that address the social and emotional aspects of health.
“I truly couldn’t do this work without our nurses,” he says. “They are essential.”
Outside of medicine, Dr. Dinn enjoys slowing down and spending time on creative pursuits. He is an avid gardener, a saltwater reef hobbyist, and a devoted Pomeranian enthusiast. He also enjoys exploring Texas Hill Country wineries and spending time outdoors.
Patients may notice that Dr. Dinn often wears blazers or sport coats rather than a traditional white coat—a choice he makes intentionally.
“They feel less intimidating for many patients,” he says. “And honestly, I enjoy them.”
When asked what advice he gives patients who want to take charge of their health, Dr. Dinn doesn’t hesitate.
“Come see me—we’ll do it together.”
Some of the most meaningful moments in his career come when patients need fewer visits because they’re doing well.
“That tells me they feel empowered,” he says. “And that’s exactly the goal.”
For patients seeking a primary care physician who listens carefully, values partnership, and treats the whole person—not just the diagnosis—Dr. Dinn offers care rooted in trust, understanding, and long-term support.
