Patients want a clinician who listens carefully, explains clearly, and takes the time to understand who they are beyond a diagnosis. For many patients, that sense of partnership begins with Tobi Leigh Pomeroy, MS, FNP BC, a Family Nurse Practitioner whose approach to care is grounded in education, trust, and genuine connection.
With more than 20 years in healthcare and 14 years as a nurse practitioner, Ms. Pomeroy brings both deep clinical experience and a steady, compassionate presence into every patient interaction. Her philosophy is simple but powerful: informed patients are empowered patients—and empowerment leads to better health outcomes.
Ms. Pomeroy’s journey into healthcare began long before graduate school. As a teenager, she watched her youth pastor work as a flight nurse after years in the emergency department.
“I thought it was one of the most exciting and meaningful careers I had ever heard about,” she recalls.
That early inspiration eventually led her to become an emergency room nurse herself, where she gained firsthand experience caring for patients in high pressure, fast paced situations. Even then, one thing was clear: she had a natural inclination to care for others.
“I’ve always had a natural desire to take care of people—and even animals,” she says.
Over time, her desire to build deeper relationships with patients and guide them through long term health decisions led her to advance her career as a nurse practitioner.
Ms. Pomeroy completed both her Bachelor of Science in Nursing and her Master of Science in Nursing, Family Nurse Practitioner track, at Texas Woman’s University. Her academic training, paired with years of frontline clinical experience, shaped a provider who is both knowledgeable and highly approachable.
She chose family medicine and primary care because it allowed her to care for patients across the lifespan—building continuity and trust over time.
“My experience in the Emergency Department exposed me to patients of all ages,” she explains. “Transitioning into family practice felt like a natural fit.”
In her day to day practice, Ms. Pomeroy focuses on health promotion, disease prevention, and chronic disease management, with a particular passion for treating hypertension and diabetes—two of the most common chronic conditions in the United States.
She frequently manages conditions such as:
She also recently completed a refresher course in joint injections, expanding the in office services she can offer for musculoskeletal concerns.
Medicine is constantly evolving, and Ms. Pomeroy makes continuing education a priority. She attends professional conferences at least twice a year and regularly reviews clinical resources,
journal articles, and current research.
“Staying up to date with evidence based practices is essential to providing the best care possible,” she says.
Ms. Pomeroy describes her care style as intentional and educational.
“I strongly believe that an informed patient is an empowered patient,” she says. “When patients understand their condition and treatment options, they’re more engaged—and outcomes are better.”
She takes time to explain diagnoses and care plans clearly, helping patients feel confident rather than overwhelmed.
Trust begins with listening.
“Taking the time to truly listen and allowing patients to ask questions is key,” Ms. Pomeroy says.
When multiple treatment options exist, she discusses the benefits and drawbacks of each and encourages patient input.
“A care plan only works if the patient is comfortable with it and committed to it.”
Patients often describe her as caring, knowledgeable, and attentive—and many say they appreciate that they never feel rushed during visits.
Ms. Pomeroy is especially passionate about caring for older adults, a population that often requires more time and attention.
“They deserve exceptional care,” she says. “I take pride in ensuring they feel heard, respected, and supported.”
She also conducts monthly home visits for homebound senior patients, helping ensure continuity of care beyond the clinic walls.
The first visit is a comprehensive “meet and greet,” focused on building a strong foundation. Ms. Pomeroy reviews medical history, medications, family history, and current concerns while getting to know the patient as a whole person.
She encourages patients to share their most important concerns early in the visit so those priorities are addressed first.
Ms. Pomeroy was drawn to WellMed’s patient centered model of care, especially its focus on seniors.
WellMed’s manageable schedules and access to care teams—including social workers and care managers—allow her to provide thoughtful, comprehensive care.
Outside of work, Ms. Pomeroy enjoys reading, exercising, traveling, and spending time with her family. She and her husband volunteer with a local bulldog rescue organization and have adopted four bulldogs over the past decade.
One thing that may surprise patients: she has been weightlifting for over 20 years and encourages strength training—especially for women—to support bone health, balance, and longevity.
Ms. Pomeroy’s advice for patients looking to take charge of their health is practical and achievable.
“Start with the basics,” she says. “Stay on top of wellness visits, screenings, movement, nutrition, and sleep.”
What she finds most rewarding is the trust patients place in her—especially when they refer friends and family.
With decades of experience and a deeply patient centered approach, Tobi Leigh Pomeroy, MS, FNP BC, offers care built on education, trust, and genuine partnership.
