WellMed Medical Management has received two prestigious designations recognizing how we provide older adults with quality, compassionate care:

  • Age-Friendly Health System: for commitment to evidence-based, high-quality care for older adults
  • Care Provider Recognition Program: for commitment to quality and preventive care

Age-Friendly Health System: The Age-Friendly Health System initiative recognizes hospitals, health systems and physician practices as “age friendly” based on following evidence-based practices, and providing what matters the most to older adults and their family caregivers. WellMed was recognized for having values aligned with those of an Age-Friendly Health System:

  • Aligning care with each patient’s specific health outcomes goals and care preferences, including end-of-life care
  • Ensuring medication aligns with patients’ goals, mobility and mental well-being
  • Preventing, identifying, treating and managing dementia, depression and delirium
  • Ensuring patients’ mobility and ability to move safely

Age-Friendly Health System is an initiative of the John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI), in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States (CHA).

WellPoint Care Provider Recognition Program: WellMed Medical Management is one of only 19 national recipients in the inaugural Care Provider Recognition Program.

The award, from managed care partner WellPoint, celebrates the top 5% of care providers based on quality performance, helping patients stay healthy through medical best practices such as:

  • Preventive care screenings
  • Immunizations
  • Chronic disease management
  • Medication adherence

The award recognizes WellMed for going above and beyond to deliver high-quality, cost-efficient care, while driving health equity and exceptional patient experiences and outcomes.
